Form is just your brshape, its the drop down beside simple circle. Judging from the screenshot, the blending tab is just a compressed version of the blending settings below the texture drop down. I don't know what version is being used in the first screenshot, or how they got that layout, but the settings should all be there. Thanks!

CREATING YOUR BRUSH: i think the difference of SAI in regard to other programs is that brushes are made from manual settings, by selecting two textures (the 2 dropdowns right bellow Density) to compose it. you can right-click on a free tool slot to create a new brush, and then fully customize it. from my understanding, the name/icon chosen.
